Since this account was previously set up with OPUS they were already in the system. On Sept 21 a new account was open but our records stated the meter was pulled. At sometime a meter
was reinstalled.

A Meter Inspection form was sent to me from Dave. It strictly stated: valves off at this time & Clay Barnett must sign off prior to connection. 

Billing requested meter info and it was placed on hold due to outstanding inspection violations.

Apparently the water has been turned on and per your request we have turn it off and placed a cable and lock on it. It will be left off until you tell us it’s ok.
