Regards, Matthew Wiatrek From: Ron King [mailto:RGK@freese.com] Sent: Friday, March 18, 2011 10:18 AM To: Wiatrek, Matt Cc: Ron King; Nancy Cline; Jimmy Darrell Dunham (jim@dunhamengineering.com); dwilde@addisontx.gov Subject: Addison EST - Qaulity Assurance Concrete Construction Operations Matthew: I have taken a look at the referenced record data you submitted, and have it Filed as Received in FNMP since it is record data. I have also attached it to this e-mail for Nancy’s, Dave’s, and Jim’s reference as they inspect the concrete construction. I do have a handful of questions/comments for you on this: 1. Item 4.1 indicates that Ron Mathis will designate a competent person(s) to perform the quality assurance measures. Please let us know who that person will be, and their qualifications/exper ience. [Wiatrek, Matt] The superintendent in charge of Landmark’s pedestal crew will serve as the competent person. The pedestal crew responsible for the Addison project is not yet confirmed. 2. Item 4.2 – please note that you need to have up-to-date as-built redlines on site throughout the project. [Wiatrek, Matt] Acknowledged 3. Item 6 – what is the status of your FAA construction permit? [Wiatrek, Matt] Discussions with Bruce Beard(FAA) have taken place. It was decided that a separate determination must be filed for the crane we plan to use during Tank Top Out and Installation of Wind Turbines. This determination was submitted on March 18, 2011. 4. Item 7.5.4 – why is plumb not checked for every lift, instead of every 5th lift? [Wiatrek, Matt] A radii report is executed daily and can be available for review. 5. Items 7.6 and 7.7 – any deviations need to be authorized by the Owner in addition to Landmark management. [Wiatrek, Matt] Acknowledged 6. Section 9 – are the following items also controlled? a. Using the same supplier/batch plant throughout pedestal construction to maintain consistent color and appearance b. Ensuring the same aggregate and source of aggregate are used throughout pedestal construction to maintain consistent color and appearance c. Ensuring the same cementitious materials and source of materials are used throughout pedestal construction to maintain consistent color and appearance [Wiatrek, Matt] Items a, b, and c above are all controlled. The PO between Landmark and our concrete supplier addresses each of these items. Also, Landmark’s competent person will visit the batch plant prior to every pedestal concrete pour. 7. Item 11.3 – Understanding that the concrete strength achieved governs the minimum form-on time, please also keep in mind the need for consistent color and appearance as related to consistency in form-on time between lifts. [Wiatrek, Matt] Acknowledged 8. When do you anticipate doing the concrete wall test panel? Are we a ways out from that? [Wiatrek, Matt] The test panel will be complete during the first week Landmark’s pedestal crew arrives on site. This is tentatively scheduled for the week of May 2. Thanks, Ron King, P.E.
