Attached is our Payment & Bills Paid Affidavit concerning the Public Water, Sanitary, & Storm Drainage installed by CalHar Construction, Inc. at the Methodist Hospital for Surgery -
17101 Dallas Parkway.  I expect you have already received our Maintenance Bond + the testing reports as needed to receive final for this project.  If you have not received these documents
or need any additional documents from CalHar to obtain a final release from the Town of Addison, please let me know.
