Keller, TX 76248 (O) 817-430-9500 (F) 817-430-9207 (M) 817-819-0703 From: Joel Massey [mailto:jmassey@icon-engineers.com] Sent: Monday, August 08, 2011 1:38 PM To: Andrew Rolph Cc: anord; Clay Barnett; ELittle@udr.com; Bruce Dunne; Rodger Harrison; Tom Harle Subject: Grade Problems on Traffic Control Cabinet Slab - Vitruvian Park Andrew, We appear to have some grade problems with the top of concrete slab around the traffic control cabinet foundation installed in the Vitruvian park Phase 1B project. As you may recall, specific grades were set for this concrete pad so that pavers could be added later when the streetscape is done. Well that time is now and the top of concrete grades appear to be 3” higher than the design grades. It actually looks like the finished top of paver grades may have been used for the top of concrete slab. We have made an effort to grade around this condition, but because of surrounding constraints, we are unable to do so. We request the following from you to correct the problem: 1. Sawcut, remove and dispose of the concrete slab around the traffic control cabinet foundation leaving the actual cabinet and foundation in place. 2. Chip away the remaining concrete around the cabinet foundation to a near flat vertical surface. 3. Notify the Town of Addison, Brandon Graham, prior to starting the work. An expansion joint around the cabinet foundation and new concrete paver base will be poured by others with the final streetscape pavers. The streetscape work is in progress and we request this work be completed as soon as possible. Please let me know if you have any questions or need any additional information. Joel A.
