Attached is Pay Application 4.  Both Change Orders were incorporated into the pay application.  Please have it executed and returned to me for payment.

Following are some change order related items:

 

Item 154 – Overage of 1 unit that is not covered in this pay application.  This needs to be included in a change order.

Item 182 – There was an overage of 12 units on the last pay application.  It was paid, but it needs to be included in the next change order.

Item 267 – There was an overage of 240 units in the last pay application and 60 units in this pay application.  The 240 units in the last pay application were paid, but the 60 units
in this pay application were not paid.  These need to be included in a change order

Item 271 – Overage of 1 unit that is not included in this pay application.  This needs to be included in a change order.

 

Future pending items:

Excavation for earthen channel

Platform for Duct Bank System

 

The total of these items is $61,610, which if you add that to my pay application, it equals the number on yours.  However, please do check it for accuracy.  Also, I am not getting the
same number for previous payments that you have.  Please check it.  In the future if there are any overages that occur, please add them to the bottom of your spreadsheet similar to the
three items already there.  It will help me get the pay application out quicker.
