Below are the comments from Utilities. Please add the new ones to the list. Some of the items may be repetitive.
1. General Note: Clean all concrete slag off Fire Hydrants. Clean Expansion Material from around all Fire Hydrants located in sidewalks.
2. 0+00.00 W-A3 North East Valve cut stack so that locking debris cap will fit.
3. 6+64.14 W-A Remove concrete from inside valve stack.
4. 6+53.14 W-A Repair Fire Hydrant leaking oil from Operating Nut and replace gaskets on Nozzle Caps.
5. 6+38.83 W-A Straighten Service Line East of the existing two service lines and center up on Meter Box.
6. 3+90.91 W-A Repair? Fire Hydrant weep holes blowing water back up through sidewalk.
7. 0+76.13 W-A Valve Stem or Valve Extension is Broken Replace or Repair.
8. 17+19.46 W-A South East 8” Valve needs to be cleaned out.
9. 0+66.07 W-A8 Replace Valve Stack lid with correct model lid.
10. 21+05.21 W-A Straighten Valve Stack.
11. 13+10.23 W-A Valve for Fire Hydrant lead needs to be cleaned out and straightened.
12. Meter box to the North East of Meter Box in Item #50 on this list needs to be replaced and set uniform with the other Meter Boxes on all four corners at intersection of Ponte Avenue
and Vitruvian Way.
13. S.S. Manhole located just South of Book Store needs to be raised to grade and grouted in.

· Locate and set to grade w/locator pads existing san. Cleanouts at SW corner of parking lot
· Replace broken curb at Andrus sidewalk to parking lot
· Replace outlet flange meter bolts Andrus irrigation meter with stainless steel
· Repair/replace broken curb at Brookhaven Townhome sign
· Clean and dress property at 3718 Vit. Way
· Finish berm at NE corner Marsh/Vit. Way
· Andres damage to AT&T handhole at Park Rd NW
· Replace curb at NW ADA ramp at ParkRd, Andrus to replace broken wing panel.
· Clean out sanitary sewer inverts STA 17+30 & 17+84
· Replace MH ring STA 19+54 Line B
· Place infil pans in all MHs in pavement.
· Clean valve stack at STA 28+50 line W-A
· Valves at fire service to Bldg 1 and S side of WA-5 tap are crooked
· Replace broken curb at water meter vault.
