Attached are two forms to be signed by Ron. The first form, Appendix A, should be signed and inserted in each of the five books. The second form should be signed as well and mailed
to the Executive Director at TCEQ as shown in the letterhead. Then a signed copy should also be inserted in each of the five books as well. Call if any questions.
