Clay on the attached NOI Alison used a Customer Number that does not match the number listed in the registry for the Town of Addison.  I show the TOA as 601725211 that covers the entire
town, so I assume this will work.

There is a $325 application fee.  The form must be signed by either the mayor or city manager.  The easiest way to complete the form is to get a copy of the contractor’s NOI and copy
all of the pertinent information from it.

 

Alison recently completed the one for Spring Valley and had looked into submitting the payments electronically, but I am not sure what the outcome of that was.

Clay Elite Landscape is supposed to provide the SWPPP for the Bush Elementary and Redding Trail Extension.  Did I understand that I also need to provide a NOI, and if so do you have
