I completed it.  The vendor number is 06138.  The form was emailed back to the vendor as I am unable to change the information submitted on the form and I have to close the form which
generates an automatic email.  In the future, please complete the new vendor form internally so we can track who initiated the new vendor and it gets emailed back internally.
