Distribution ofMeeting Minutes from the June 18, 1998 Meeting Leslie Sagar distributed the meeting minutes, 2. Comments on Draft Meeting Minutes from the June 25, 1998 Meeting Leslie Sagar stated that she had faxed copies of the draft meeting minutes to Jim Gardner, Jim Pierce, and Zane Ryan prior to the meeting. The only comment was from Zane Ryan regarding item number 6, Coordination with the FAA Tower. Zane stated that the minutes should reflect the fact that Sam Stuart authorized the aircraft departure when the airport was closed without anyone else's knowledge. 3, Contractor's Quality Control Program and Testing Plan Leslie Sagar stated that once again the Contractor's testing lab, Terra-Mar, did not send a field technician to perform tests on the concrete that was placed starting at 2:30 am July 2. Due to their absence, Julian Wise directed Maxim, the Owner's testing lab, tu perform concrete temperature, slump, and percent air testing on each sublat ofthe concrete that was placed. Leslie Sagar reminded the Contractor oftheir responsibility to adhere to their Quality Control Program. Jim Gardner stated that he would discuss the matter with Terra-Mar and that Terra Mar had apparently gotten the West Taxiway project confused with the tunnel project, Wayne Crabtree stated that he also had a concern about Maxim. According to Wayne, Maxim picked up the concrete beams that had been made on June 25 (2:30 a.m. pour) later that same day. Wayne also said he observed the Maxim technician remove beam forms from beams in the back of their truck so that they could use the forms to make up the beams for the taxiway project. Leslie Sagar stated that she would contact Maxim to discuss proper field procedures, and that she should be contacted immediately if any other unusual field testing procedures were observed. 4, Project Schedule Majed Limam distributed copies ofthe two-week look ahead and explained the proposed activities. The detailed schedule for the night work was discussed. Majed indicated the Joint L Overlay would occur on July 7, and the schedule was amended to show this element ofwork. The runway will be closed at night from 9 pm to 7:30 am on Monday July 6 thru Thursday July 2 (through Friday morning, 7:30 am). ADDISONAIRPORT Construct West Side Taxiway and Drainage Improvements Construction Progress Meeting Minutes -July 2, 1998 Page 2 Wayne Crabtree expressed concern that when night work is scheduled, the runway is not always closed promptly at 9:00 p.m., and that there were occaisions when it did not get closed until 9:20 p.m. and 9:25 p.m. Kurt Horn stated that he understood the Contractor's concern and that he would ensure the runway is closed at 9:00 p.m, 9: I 0 p.m. at the latest.' 5. Construction I Shop Drawing Submittals by Contractor Leslie Sagar distributed a summary log ofthe Contractor's submittals to date and their review statos, Majed Limarn handed in Submittal No, I04D on the P-50 I Mix Design beam breaks. Leslie Sagar asked if there were any other submittals that would be forthcoming. Majed Limam indicated that he did not foresee any others at that time. Leslie Sagar asked about the submittal on the dowels that were being used. Majed Limam stated that he would review their submittal status. 6. Requests for Information (RFI's) Leslie Sagar distributed the updated REI log. Since the last meeting, only one REI was received from the Contractor, REI No. III regarding Type H Taxiway Inlets. Wayne Crabtree stated that they want to excavate and pour concrete without using forms in the lower portion of the excavation. Tex Schmidt stated that he wanted the Contractor to use forms, and Leslie Sagar stated that she agreed. Majed Limam submitted REI No. 112 during the meeting, which pertains to the asphalt detour between Driveways I and 2. Jim Gardner stated that there is a conflict in locating the detour as shown on the plans and that they plan to lay the asphalt during nightwork on Tuesday, July 7. Leslie Sagar and Tex Schmidt agreed to field inspect the matter after the meeting. 7. Coordination with FAA Tower Al Devane reported that there were no coordination problems during the past week; however, there was another incident ofan aircraft departing after the airport/runway was closed on Sunday, June28. A Learjet transporting organs for transplant departed to Chicago. Al Devane requested that AATI contact the Tower by radio each night to advised them that the runway is closed. Kurt Horn and Zane Ryan agreed to do so. Al Devane also requested that the Contractor lock-out the runway lights each night that night work is being perfonned to ensure that the Tower cannot tum the lights on, which will also help in deterring aircraft from requesting and receiving approval to depart after the runway is closed. Also, to better coordinate with the FAA Tower, Jim Gardner agreed that they will provide a 5 to 7 day detailed look-ahead schedule. DBE Participation The Contractor is in the process of identifying additional DBE subcontractors to achieve the 15% goal and will provide documentation to URSG by July 9. 8 ADDISONAIRPORT Construct West Side Taxiway and Drainage Improvements Construction Progress Meeting Minutes -July 2, 1998 Page 3 9. Other Items a) Leslie Sagar expressed serious concern about aircraft departures occuring when the airport/runway is supposed to be officially closed, particularly since two incidents have occurred within a week period. Al Devane also expressed concern that pilots have been authorized by AATI after the Tower has advised the pilots that they do not have permission to depart. b) Leslie Sagar asked about the status of removal of stockpiled materials adjacent to the drainage pipe construction area. Kurt Hom and Al Devane both expressed concern about the continued presence of the large stockpile, and the potential hazard to aircraft safety. Wayne Crabtree stated. that the stockpile is scheduled to be moved on Monday morning, July 6. c) Kurt Hom asked ifthe night work will be completed on schedule. Wayne Crabtree indicated that they would be completing it on schedule. d) Leslie Sagar asked Jim Gardner about the status of Change Order No.s 1 and 2. Jim stated that they were being processed in San Antonio, where H. B. Zachry has its headquarters. e) Wayne Crabtree indicated that they may perform the work associated with Phase 3 during night time hours. Leslie Sagar stated that there were financial implications to the Town ofAddison that would have to be discussed/resolved ifthey were to do so (e.g. RPR services and Quality Assurance Testing budgets were developed based on day work, not night work). 10. There were nO comments other than to correct the pager number for Zane Ryan. 3. PrQject Schedule Majed Limom distributed copies of the two-week look ahead and explained the proposed activities. The detailed schedule for the night work was discussed. Leslie Sagar proposed that Majed send out a fuxlnote to URS Greiner, Julian Wise, AA TI, Maxim, and Terra Mar before 5pm each day regarding the Contractor's work program for that evening so that key penple will be kept apprised of the !ast minute changes in the work schedule. After discussion, this was an agreed upon solution to the scheduling issues. AATI agreed to notify the FAA Tower as required. Based on the Contractor's schedule (copy attached), the runway will be closed at night from 9 pm to 7:30 am on Sunday June 29 thru Thursday July 2 (through Friday morning, 7:30 am). Construction /Shop Drawing Submittals by Contractor Leslie Sagar distributed a summary log ofthe Contractor's submittals to date and their review status. The P-501 submittal is due following the 28-day beam breaks. Leslie Sagar advised that the P-605 submittal was not approved due to non-conformance with the job specifications. 5. Requests for Information CRFI's) Leslie Sagar distributed the updated RFI log. Since the last meeting, no new RFI's were received from the Contractor. 6. Coordination with FAA Tower Al Devane reported that there were no coordination problems during the past week. Jim Gardner mentioned that on Tuesday, June23, at Ilpm a pilot requested that he be allowed to take off, even though the runway was closed. The pilot did depart to the north. Al Devane said he was not aware ofthe incident and that he would look into what had occurred. Zane Ryan stated that he was not aware of AATI having authorized anyone to arrive or depart aircraft when the airport is closed; however, Zane later advised that he had learned that Sam Stuart had authorized the departure. DBE Participation The Contractor is in the process of identifying additional DBE subcontractors to achieve the 15% goal and will provide documentation to URSG. 8. Other Items a)The Contractor's testing lab, Terra-Mar, did not send a field technician to perform tests on the concrete that was placed starting at 2:30 am June 25. Due to their absence, Julian Wise directed Maxim, the Owner's testing lab, to perform concrete temperature, slump, and percent air testing on each sublot of the concrete that was placed. Leslie Sagar reminded the Contractor of their responsibility to adhere to their Quality Control Program. Jim Gardner stated that he would discuss the matter with Terra-Mar. b )Driveway 10 has a small section ofexisting concrete remaining between the cut for the drainage pipe and where the new concrete section begins. Leslie Sagar asked Jim Gardner for a cost to remove the existing concrete. Jim Gardner stated that he would review the matter and provide a cost. c) Leslie Sagar advised that apparently the contract agreement between the Town of Addison and H.B. Zachry is 28 cents more than the amount on the bid tabulation for Alternates I and 3. Therefore, Change Orders I and 2 do not accurately reflect the actual contract amount. Jim Gardner stated that he thought the home office would sign Change Orders I and 2 despite the discrepancy. Leslie Sagar stated that any future Change Orders would include a correction and a note of explanation regarding the change to the original contract amount. d) In light of historical experience associated with the KboomTown festivities, The Contractor was advised to secure equipment and inlet holes, etc. prior to the upcoming July 3rd event. B. Zachry. Two such incidents occurred, the first on Tuesday June 23 at approximately II p.m., and the second on Sunday, June 28 after runway closure had occurred, despite the fact that a NOT AM had been issued by Addison Airport of Texas, Inc. (AA TI), and that H. B. Zachry had construction equipment on the runway or within the runway safety area. Based upon our knowledge ofthe fucts and circumstances regarding regarding these two incidents, it is unclear to us why these departures were allowed to occur and who, if anyone, authorized them to occur. It is our understanding that AA TI is responsible for the management and operation ofthe Airport, yet it is unclear whether or not the Airport was formally "re-opened" by AA TI to allow these departures. URS Greiner, Inc., as the Engineer for the West Taxiway Project is not responsible for management of Airport operations. As your Engineer, we recommend that the Town of Addison and AATI conduct an immediate review of the Airport's Operating Procedures to clarifY and confirm the procedures for coordinating and communicating with the FAA Tower and others regarding runway closures. Comments on Draft Meeting Minutes1"rom the June 11. 1998 Meeting Leslie Sagar stated that Tex Schmidt bad faxed copies ofthe draft meeting minutes to Jim Gardner, Jim Pierce, and Zane Ryan prior to the meeting. There were no comments 3. Project Schedule Majed Limom distributed copies ofthe two-week look ahead and explained the proposed activities. Upcoming night work was discussed. The Contractor also agreed to provide a more detailed breakdown of the specific work elements associated with the night work. Night work will be perfurmed from 9 p.m. to 7:30 a.m., week nights only, during which time the airport will be closed. Jim Gardner indicated that they will probably not be performing night work on Friday, July 3 due to the holiday. Kurt Hom expressed concern that the night work was shown on the schedule to start on June 22 and run through July 15. After diseussion itwas agreed that Majed will correcttheschedule to show the finish date for night work to be July 11. In the event ofweather delays, the schedule will be adjusted accordingly. Nancy Wise expressed concern about providing RPR services during both day time and night time operations. After discussion it was agreed that additional manpower will be required starting on Wednesday, June 24, when night time paving commences. Wayne Crabtree expressed concern about their ability to sawcut the joints in the concrete in accordance with the project specifications (as soon as the concrete has hardened sufficiently) since the airport will be re-opened to traffic by 7:30 a.m. After discussion regarding the potential problems associated with delaying the sawcutting operation to the next night time work period, and coordination with the FAA Tower by Steve James, it was agreed that the sawcutting operation (truck, sawcutting equipment and equipement operator) could occur adjacent to the runway during the daytime hours provided that the sawcutting personnel are in contact with the Tower. Since the last meeting, two RFI's were received from the Contractor and processed (l09 and 110). 6. Coordination with FAA Tower No problems/incidents were reported during the past week. For upcoming oightwork, only AATI will have the authority to open and close the runway and issue NOTAMs. DBE Participation The Contractor is in the process of identifYing additional DBE subcontractors and will provide documentation to URSG. 8. Other Items a) Skip Norton stated that on Monday, June 22 they will need to tnm offthe power to the lighting system at 9 p.m. so that they can begin electrical work. Kurt Horn stated that the Contractor and their Subcontractors are responsible for following the lock-out procedures for tnrning offthe power, and they are also responsible for demonstrating that the lights work each morning when the power is tnrned back on. Distribution ofMeeting Minutes from the May 14. 1998 Meeting Tex Schmidt distributed the meeting minutes. 2. Conunents on Draft Meeting Minutes from the May 21 1998 Meeting Tex Schmidt had :fu.xed copies ofthe draft meeting minutes to Jim Gardner, Jim Pierce, and Zane Ryan prior to the meeting. No comments were made regarding the May 21, 199& meeting minutes. 3. Project Schedule Tex Schmidt stated that the Contractor's baseline schedule was disapproved because some activities were shown as being behind. Majed Limom stated that he would revise the schedule. Majed distributed copies of the two-week look ahead and explained the proposed activities. Paving of detour between Driveways 9 and 10 is to be done tomorrow. 4. Contractor's Request to Moddy Project Phasing Much discussion ensued about Contractor's request. If approved HBZ stated that they would like to displace threshold Saturday, May 30. AATI and URSG stated that there was not enough notice to airport users. After much discussion and exploring ofalternatives, URSG denied Contractor's request. 5. Status of Cost PrQPosals from Contractor Regarding ModjficatiQP to Runway Closure Period for Taxiways E and F Addison City Council approved proposal for night work. URSG will prepare change order for approval. HBZ \vill revise schedule to show proposed night-time work. 6. Construction I Shop Drawing Submittals by Contractor Tex Schmidt distributed a summary log ofthe Contractor's submittals to date and their review status. URSG approved HBZ's revised QC testing program, but disapproved schedule, which will be revised anyway because ofnight work. P-209 submittal was also disapproved, although URSG did consider use of crushed concrete as aggregate. 7. Requests for Infonnation fRF1'sl RFI No. 105 was submitted by HBZ and responded to by URSG during the past week. It dealt with the concrete encasement of the existing force mains. Coordination with FAA Tower No problems/incidents were reported during the past week. FAA representatives were late to the meeting and were cautioned that important information was being discussed about the Contractor's activities on the airfield. 9. Other Items HBZ will submit a value engineering proposal concerning the reuse ofP-209 base. 10. Next Meeting Date Leslie and/or Tex are unavailable next Thursday. The next meeting will be held on Tuesday, June 2, 1998 at 10:00am at the Addison Service Canter. Concerning Greiner's Meeting Minutes for Construction Progress Meeting No.5 (May 28), please note that Zachry understands the following items and events took place: Greiner's Item 4: a,} Greiner stated that Zachry's 2-Phase proposal would be formally approved, Zachry said they wanted to displace the threshold on May 30, AATI said that they needed more time to allow a NOTAM to be distributed, and Zachry said they could delay it until Monday, June l. b,} AATI explained that June I was too soon to displace the threshold, and it was decided that the FAA should be called into the meeting to share their input. Also, in response to Greiner's statements that there was now insufficient time for notification, Zachry pointed out that their 2-Phase proposal had been submitted almost a month prior to this approvaL c,) The FAA and AATI agreed that displacing the threshold during the summer months, as per the 2-Phase plan proposal, would be preferred over displacing the threshold during September and October, The COncern was the shut down of the ILS during the threshold displacement. Also, the FAA said the ILS was already scheduled to be shut down for a week in June. d,} As stated in Greiner's draft notes, there was much discussion concerning the date of the displacement. However, a solution was finally worked out at the meeting whereby the threshold could still be displaced on June 8, or later, e,) After this solution appeared to be agreed upon by all, Zachry asked Greiner iftheir previous request for cost participation by the Town was still being considered, Greiner stated that absolutely none would be considered, Discussion was had on whether two displacements would be necessary. " IDTAL P.03 EXHIBIT A SCOPE OF SERVICES FOR THE RESIDENT PROJECT REPRESENTATIVE ADDISON AIRPORT CONSTRUCT WEST TAXIWAY AND CONNECTORS WITH DRAINAGE IMPROVEMENTS (SOUTH SEGMENT) A. GENERAL The Resident Project Representative (RPR) as the Engineer's agent, wi!! act as directed by and under supervision of the Engineer, and will confer with the Engineer regarding the RPR's actions. The RPR's dealings in matters pertaining to the on-site work shall in general be only with the Engineer and the Contractor, keeping the Owner/Sponsor advised as necessary, and dealings with the subcontractors shall only be through or with the full knowledge of the Contractor. Written communications with the Owner/Sponsor will be only through or as directed by the Engineer. B. DUTIES AND RESPONSIBILITIES OF THE RPR The RPR shall: 1. Be thoroughly familiar with the plans and specifications as th.ey apply to the work being inspected, and review them frequently. The RPR must be capable of immediately recognizing if the work conforms to the contract requirements. 2. Provide full time services at all times the Contractor conducts activities on the construction site. 3. Serve as the Engineer's liaison with the Contractor, working principally through the Contractor's superintendent and assist in understanding the intent of the Contract Documents, and assist the Engineer in serving as the Owner's liaison with the Contractor particularly when the Contractor's operations affect the Owner's on-site operations. 4. Review the progress schedule, samples schedule of Shop Drawing submittals and schedule ofvalues prepared by the Contractor and consult with the Engineer as to the acceptability ofsuch schedules. 5. Review, rejection and correction ofdefective work, inspection and testing: i) observe, review and test the Work throughout the construction administration phase, as needed, to determine if the Work is proceeding in accordance with the Contract Documents. ii) reject, require correction, or uncovering of the Work for observation, or require special testing, inspection or approval of the Work that in the opinion ofthe RPR does not confoml to the Contract Documents or is defective, unsatisfactory, or faulty, and notify the Engineer and Owner in writing ofthe reason for each such rejection, correction, and uncovering. The RPR shall evaluate, if requested by the Owner, the impact of each such rejection, correction, and uncovering of the Work on the Contractor's scheduling. iii) verify that tests, equipment and systems startups and operating and maintenance training are conducted in the presence of appropriate personnel, and that the Contractor maintains adequate records thereof; and observe, record and report to the Engineer appropriate details relative to the test procedures and startups. i v) accompany visiting inspectors representing public or other agencies having jurisdiction over the Project, record the results ofthese inspections and report to the Engineer. 8. Maintain at the job site orderly files for correspondence, reports ofjob conferences, Shop Dra\vings and samples, reproductions of original Contract Documents, including all Work Directive Changes, Addenda, Change Orders, Field Orders, additional drawings issued subsequent to the execution of the Contract, Engineer's clarifications and interpretations of the Contract Documents, progress repotis, and other project related documents. 9. Keep a diary or log book, recording Contractor hours on the job site, weather conditions, data relative to questions of Work Directive Changes, Change Orders or 2P:\DOCS\SUBAGREE\RPR\EXUlnITA.WPO changed conditions, list ofjob site visitors, daily activities, decisions, observations in general, and specific observations in more detail as in the case of observing test procedures, send copies to the Engineer, and provide copies to the Owner upon request. 10. Record names, addresses and telephone numbers of all subcontractors and major suppliers of materials and equipment. II. Furnish the Engineer with weekly reports as required ofprogress of the Work and of Contractor's compliance with the progress schedule of Shop Drawings and sample submittals. 12. Consult with the Engineer in advance of scheduled major tests, inspections or start of important phases of the Work. i) draft proposed Change Orders and Work Directive Changes, obtaining backup material from the Contractor and recommend to the Engineer Change Orders, Work Directi ve Changes, and Field Orders. 13. Report immediately to the Engineer and Owner upon occurrence of any accident. 14. Review applications for payments with the Contractor for compliance with the established procedure for their submission and forward the recommendations to the Engineer, noting particularly the relationship of the payment requested to the schedule of values, Work completed, and materials and equipment delivered at the site but not incorporated into the Work. The Engineer shall then determine the amount owing to the Contractor and shall certify requests for payment in such amounts, on the basis that the Work has progressed to the point indicated and that the quality of the Work is in accordance with the Contract Documents. C. LIMITATIONS OF THE RPR'S AUTHORITY The Resident Project Representative: 1. There were no further comments. 2, Comments on Meeting Minutes from the April 30. 1998 Meeting Draft Minutes were faxed to meeting attendees on May 4, 1998, meeting minutes, There were no comments on the 3, Pmject Schedule Ms, Sagar advised that the Contractor had submitted a schedule on May I, 1998; however, the schedule assumed two construction phases instesd of three as per the contract documents, Ms. Sagar requested that the Contractor submit a project schedule which confurms with the contract documents. Jim Gardner stated that they would submit one by May 8, 1988. Ms. Sagar asked for tile 2-week schedule ofactivities. The Contractor did not have one prepared for the meeting, Ms. Sagar advised that the Contractor needs to provide a 2-week schedule at each ColllltrUction Progress Meeting so that everyone will have a clear understanding ofthe Contractor's work activities during that period and so that AATI can properly advise tenants in advance of uncoming work activities. Inlieu ofa 2-week schedule, the Contractor's project schedule based on two phases was reviewed and the Phase I work activities were discussed. The Contractor was advised by Ms, Sagar that the work activities related to Phase 3 could not proceed until the Contractor formally submitted a request to alter the phasing plan and a Change Order is processed, Jim Gardner then submitted a letter dated May 2, 1998 requesting a change to the phasing plan. Ms. Sagar stated that it would be reviewed, 4, Status of Cost Proposals ITom Contractor Regarding Modification to Runway Closure Period for Taxiways E and F Mr, Gardner submitted a letter dated May 5, 1998 with cost proposals for accomplishing work in one weekend, and also during multiple nights, that was to be perfonned in two weekends. Ms. Sagar stated that it would be reviewed, 5. Mr. Gardner stated that they had not yet obtained the Building Permit or the licenses. 6. Submjttals by Contractor Ms. Sagar stated that the Contractor had submitted some information regarding the Quality Control Program but that in lieu of submitting each piece individually, the Contractor should combine the various components inte one submittal document. Jim Gardner stated that they would do so. Ms. Sagar reminded the Contractor that they needed to submit information previously requested regarding their DBE subcontractors. Ms. Sagar distributed a summary log ofthe shop drawing and other submittals received as of May 6, 1997 from the Contractor and discussed the status ofthe submittals as well as some ofthe general review comments. Jim Pierce requested that all anticipated submittals be included on the log. 7. Requests for Information (RFI's) Ms. Sagar distributed a log ofthe RFI's received as of May 6, 1998 from the Contractor. Regarding RFI No, 101 pertaining to the request fur installation/use of an access gate at the north end of the prqject site, Ms. Sagar stated that a written response was forthcoming and that permission would be granted provided that the Contractor be responsible for maintaining the existing underground cables through a period extending 6 months beyond the end ofthe construction project, After discussion which led to the understanding of a concern that the direct buried cables need to be protected from "slicing" caused by heavy truck traffic on the surface, it was agreed that the Contractor would only be responsible for maintaining the cables through the end ofthe project provided that he adequately "bridge over" the cables, Subsequently, it was decided that the cables could be better protected by enclosing them with a split duct and concrete encasement. Regarding RFl No. 102 pertaining to the electrical load calculations and data on the existing lighting system, Ms. Sagar advised that a written response would be forthcoming and was expected the next day by the electrical engineer responsible for the electrical design. Wayne Crabtree stated that they would be submitting an RFI regarding the stonn sewer in the vicinity of the temporary detour at Driveways 9-10 since construction of the detour will cut off storm drainage south ofDriveway 9. 8. Coordination with FAA Tower Regarding Safety on the Aimort During Construgtjon Ms. Sagar stated thattheprevious meeting which started at 8:30 a.m. with FAA personnel addressed the issue ofthe importance of Contractor coordination and communication with the Tower. Zane Ryan advised the Contractor that helicopters are not on ground frequency and have been known to hover and land in the vicinity of the project area, and that the Contractor should be on the alert for helicopter activity. Other Items a. FAA Cables -Steven James (FAA) stated that the communications cables cannot be spliced, and ifthey are damaged the 25-pair control cables must be replaced. There are two runs: 1) Tower to Glide Slope, and 2) Glide Slope to Localizer. The depth of the buried cable ranges from 24" to 48" below grade. b.
