During Construction Progress Meeting No.1, Zachry brought up the issue of the proposed electrical system creating' a possible overload 011 the existing electrical system. RFI-I02 (April 29, 1998) conceming this potential problem was submitted to URS Greiner and calculations were returned to us, later. While showing that the existing system is adequately designed to handle the load, the conditions of the existing system could not be detern1ined. In fact, AATI stated that they frequently experience severed wire and cut, or 'skinned', insulation on their existing buried wires. The native rocks found tlu'oughout the sUlTounding ground within the airport was mentioned as a potential cause. 2. Our system was tested before tying in the existing circuit and tested well. After certain existing l'lmway-Iights failed to burn on the night in question, we thoroughly re-tesled our circuits and found everything tested 'good' again. We continued to test the existing circuit until late that night before finding a short-circuit in the AA 1Ts existing lighting circuit. 3. The problems experienced by the AATI on the night in question, are a design issuenot a constlllction issue. Our personnel spent time in a good-faith effOli to locate the airport's existing faulty wiring. We respectfully deny your request for reimbursement of $1,578.29 and ask your assistance in rccovcling the $750 in costs, we inClined while assisting the. airport in locating the defect in their system. We are providing you with one (I) mylar, two (2) blackline prints, and one (I) electronic file copy of the revised ALP. Revisions were made to the November 1997 ALP prepared by your Master Plan consultant, Bamard Duukelberg, as follows: 1) recent construction improvements to Taxiway B and the new connector taxiways; and 2) a correct depiction of the Glide Slope Critical Areas associated with the Instrument Landing Systems (ILS) at each runway end since these critical areas were discovered to be incorrectly shown on the ALP as documented in my letter to you dated November 19, 1998. The Glide Slope Critical Areas were revised to show an x-dimension of 1300 feet in accordance with FAA Order 6750.16B, Change 2 for a Capture Effect glide slope. These two revisions to the ALP drawing are noted in a revision block on the drawing. It is recommended that the Town of Addison submit the revised ALP to Bruce Ehly of TxDOT Aviation Division for coordination and FAA approval. We recommend that you advise Mr. Ehly that when the Glide Slope Critical Area discrepancy was discovered, the FAA Airways and Facilities Sector Office at Addison indicated that they had not seen the November 1997 ALP, even though the ALP was approved by FAA Southwest Region. It is also suggested that the Town may \vish to further update the ALP with respect to the Keller Springs Rd. Tunnel project. The tunnel is shown on the drawing; however, we believe that a number ofhangars and buildings are shown on the ALP which were removed by the tunnel project subsequent to November 1997. Please let me know ifyou would like for us to undertake the updating ofthe ALP to reflect changes in airport features associated with the tunnel, and we will be happy to provide you with a fee proposal. Box 144 Addison, Texas 75001 Dear Jim: We understand that H. B. Zachry is about to finish their job at the Airport and is seeking reimbursement for two different items. The work stoppage at the start of the project and Zachry's failed attempt to restart the lighting system November 20th, 1998 are the two items I understand they are seeking reimbursement for. First, the work stoppage in May, was the result of Zachry leaving a taxiway in a condition that was impassable by even aircraft under tow.. The taxiway millings had been left in place and aircraft under tow were skidding because of chunks of asphalt wedging underneath the tires. Clearly, the April3td, 1998 minutes states on page 3, the second line, "The Contractor will maintain aclean, active taxiWay." Ms. Leslie Sager, with Greiner was concerned because we stopped the construction, however, neither Zachry nor Greiner are professional airport managers. In their unqualified opinion, the potential damage was not an imminent threat to personal safety. We disagree with the whole premise. It is our responsibility to protect both the peaple and their praperttj while on-airport and we did. Clearly, the aircraft landing gear under tow could have collapsed and cost far in excess of the $3,000 loss time estimate submitted by Zachry. We were not only doing what we are tasked with through our Operating Agreement with the Town but what the minutes dearly state as a SAFETY ISSUE. It is also worth noting the next day, all the spoils from the milled taxiway were moved so that safe passage could once again take place. Greinefs decision to call our judgment into question has led to this submittal from Zac and is both offensive and a demonstration of reme s poor an . g of the issue as the project administrators. Secondly, we made it dear at the start of the project that the lighting system was fully operational and that Zachry would be responsible for any and all maintenance to the lighting system throughout the duration of the project and it was again reflected in the June 11th, 1998 minutes. There was a system inspection with Skip Norton to demonstrate the system was operational with Zane Ryan of AATI. On the evening of Friday, November 20th, my staff received a phone call from Alvin DeVane around 4505 CLAIRE CHENNAULT. ADDISON, TEXAS 75001 • 9721248·7733 Mr. James Pierce December 21, 1998 Page 2 5:45 PM stating that the lighting system was not operational and they wanted to know whether to continue aircraft operations with no lighting. The Airport was in fact shut down at approximately 6:15 PM because of the lack of lights. After contacting Jim Gardner, he stated the lights at the south end of the Airport had been down all day and that his guys could not get them all working again. Jim Gardner 11§1!f! notified us of the problem he was having with the lighting system. He stated his guys could not resolve the outage and that he was going to pull his men off the job for the night. We promptly called Walker Engineering, Inc., our airport lighting professionals. Walker came out, identified the problem and corrected it. Attached is the bill for their 􀁾􀁥􀁲􀁶􀁩􀁣􀁥􀁳􀀮􀀠We are requesting that the Town pay this bill and collect from Zachry per the agreement. Additionally, we would like to see Zachry held accountable for the liquidated damages as specified in the Addison Airport Construct West Taxiway and Connectors Contract, Sec. 9.0 LiqUidated Damages, Para. 9.2, the third paragraph. The Airport was closed because of the lighting situation from 6:15 PM until 6:30 AM which is twelve hours and fifteen minutes. The damages, as I understand them to be, are $9,750 in liquidated damages plus the Walker Engineering bill for the repair of $1,578.29. Of the three FAA funded Airport Improvement Projects at Addison Airport, this by far has been the most difficult when it should have been the easiest. My frustration has not only been with the construction contractor but with the project administrators who have repeatedly defended construction concerns over the operational necessities of the Airport. B. Zachry intends to file claims for the work stoppage ordered by AATl on May IS, 1998 and also for work in relation to a runway lighting failure that occurred on November 20, 1998. To date H. B. Zachry has not filed any claims for these items. Based on recent discussions with H. B. Zachry personnel, we do not anticipate that any claims will be filed. 2) Work Stowage on May IS, 1998 We do not concur with Mr. Hom's depiction of the work stoppage event that occurred at approximately 10a.m.onMay 15, 1998. Ata 1:30p.m. meeting that day it was determined that the primary cause for AATl's concern regarding the contractor's activities was due to the fact that they did not understand the contractor's work schedule, even though it had been discussed in Construction Progress Meeting No.2, held on May 14, 1998. The contractor had constructed a temporary pavement tie-in to Driveway No. 10, serving Building No. 92, and aircraft were being towed to and from the hangar without incident. The contractor's activities and the aircraft towing were monitored by a full-time resident project representative with extensive airport construction experience. AlthoughAATl personnel became concerned about the towing operation, it was determined that AATI could have contacted the FAA Tower and advised them to re-direct aircraft along a different route or cease the towing operation in lieu of instructing the contractor to stop work. We do not dispute AATl's authority or responsibility for the safe management and operation ofAddison Airport. Inthis particular instance however, we believe less extreme measures were warranted. 3) Runway Lighting Failure On Novernber 20, 1998, the contractor had turned offthe power to the runway and taxiway lighting system in order to make the fmal electrical connections at the handholds to the URSGreiner taxiway lights and signage inside the safety area. After completing his work, the contractor turned on the power and the taxiway lights and signage came on. The runway lights came on partially and then went out. The contractor then proceeded with trying to determine the source ofthe problem. The contractor advised our full-time project representative that they would not leave until the problem was located. The contractor checked each runway light, proceeding north. At approximately 1000 feet from the north end of the runway, far away from the West Taxiway construction area, the problem was discovered and the contractor remained on-site working with Walker Engineering personnel until 2 a.m., at which time the lights were restored. AATI contacted Walker Engineering and requested their services. At no time did AATI contact URS Greiner personnel to advise us ofthe runway lighting problem or to discuss the probable cause or responsibility for the runway lighting failure. Itis possible that the runway lights may have been affected by the work associated with the taxiway lighting; however, it is equally probable that the runway lights could have failed in any event, regardless ofthe taxiway lighting work. Thus, it is difficult to discern whether the contractor should be held responsible for repair costs associated with the runway lights or for liquidated damages associated with the runway closure. It is our opinion that an assessment of liquidated damages would probably prompt the contractor to file a delay claim for the work stoppage on May 15, as well as for the lack oftimely nighttime runway closures by AATI for work performed in the runway safety area. We do not believe a protracted claims dispute is in the best interests ofthe Town ofAddison. In order for any airport construction project to be successful, all parties involved must be willing to work together in a cooperative manner. Unfortunately, it was our observation that AATI was frequently absent or late to the Construction Progress Meetings, they did not coordinate with the tenants on the west side as they had agreed to do, they made last minute requests that work be performed at night which resulted in costly change orders to the Town of Addison, they have no written airport operating procedures, and most significantly, on several occasions they allowed aircraft to depart when the runway was officially closed and occupied by the contractor. The ILS sign locations were determined based on the Glide Slope Antenna Critical Areas depicted on the Airport Layout Plan (ALP) dated June 1997 which was prepared by Barnard Dunkelberg. The June 1997 ALP was also included in the constrUction drawings when the West Taxiway project was advertised for bidding in December 1997. After receiving comments from Kurt Hom ofAATIthat the FAA Airways and Facilities stafflocated at Addison Airport had concerns regarding the ILS sign locations, we began investigating the comments. We first learned that a later version of the ALP had been prepared by Barnard Dunkelberg in November 1997, and we understand thatthis later version ofthe ALP was ultimately approved by the FAA Southwest Region. Thinking that perhaps the Glide Slope Critical Areas had changed between the June 1997 and November 1997 versions ofthe ALP, we obtained a copy ofthe November 1997 ALP and made a comparative review. We found the Glide Slope Critical Areas to be of the same dimension and in the same locations on both ALP's. We then made a careful review of the Glide Slope Critical Area dimensions depicted on the both ALP's and determined that they did not conform to the standard location/dimensions for Glide Slope Critical Areas presented in Chapter 6 of the FAA Advisory Circular on Airport Design, AC 15015300-13. We contacted Mr. Curtis Gray at the FAA Airway and Facilities Field Office at Addison Airport and he provided us a copy of selected pages from FAA Order 6750.16B Change 2, dated May 26,1989, which provided us with the specific dimensions for both Glide Slope Critical Areas at Addison Airport. At your request and as a professional courtesy, I contacted Ryk Dunkelberg ofBarnard Dunkelberg and advised him ofthe disparity regaromg the dimensions ofthe Glide Slope Critical Areas on the June 1997 and November 1997 ALP's. I subsequently faxed him the information we had received from FAA Airways and Facilities. I recommend that the Town of Addison contact Barnard Dunkelberg to request a correction ofthe ALP and re-submittal to FAA Southwest Region. An incident occurred approximately 6:00 am October I when a business jet, owned be an airport tenant, over-ran the relocated threshold despite notification fj-om AATI, NOTAMs, ATIS broadcast, markings, and threshold lights. Kurt Hom has the tail number and has been communicating with business owner. He is considering contacting FAA FSDO. HBZ will now place barricades on runway pavement each night. Barricades will be placed with at least 50 feet between them to allow an over-running aircraft to pass between them. 8. Other Jim Gardner will provided cost for additional night work. The material and its use are described in Chapter 15. Its advantages for repair work are its low shrinkage and tbat it can be successfully placed under water. STRUCTURAL REPAIRS USING EPOXY RESIN Of the large variety of epoxy resins available, use only those fomlulated specifically for concrete repairs. Normally these will be two-component, 100 percent solids, epoxy resins with no diluents or nonreactive Uquids. Epoxy resin materials are furnished in different grades, ranging from the unmodified binder nlaterials with low viscosity through tbe various grades of grout. Grout ranges from high Viscosity fluids to trowelable pastes and gels formed by the addition of thickeners or nne mineral fillers. The inspector is governed by the requirements of the contract documents in the selection of epoxy resin Dlaterials for repairs. In the absence of contract document requirements, consult the designer for requirements and instruction. ASTM C 881 has been developed to cover the various types of epoxy resin used for bonding concrete. Information given bere is only for background data and additional guidance. At times, materials otber than epoxy resins, particularly latex and polyester materials, are also used as bonding agents and as admixtures to increase the flexural strength and elongation of portland cement mortars for repairs. Detailed guidance for the use of epoxy resin materials in concrete construction, inclUding repair work, is available in tbe guide prepared by ACI Committee 503, and in four ACI standard specifications (ACI 503.1, 503.2, 503.3, and 503.4). Additional data can be found in References 5 and 31. Consult these references before using epoxy resin in concrete repair work. Moisture and temperature sensHlvlty Early epoxy resins were sensitive to moisture, both during cure and, to a much lesser extent, after hardenlng; and many presently available products stin are. Many sucb materials are good products, but tbeir use must be restricted to loca tions where they will not be subjected to moisture eitber before curing or for long periods after curing. Epoxy resins now available tbat are insensitive to ntoisture can be used for applications When it is impractical to completely dry the base concrete, for bonding fresh portland cement concrete, and for locations tbat will be under water in service. Epoxy resins bond to most construction materials. Specialized products tbat bond to PVC and to neoprene can be used for concrete repairs involving water stops. However, epoxy resin does not bond to materials such as polyethylene, Teflon, and greased or waxed surfaces. Polyethylene and beavy waxed paper are used for form Iinlng for epoxy resin repairs. Hardened epoxy resin has a much higher thermal coefncient of expansion tban concrete. Unless compensated for, aU epoxy repairs tend to crack and come unbonded under expansion-contraction cycling produced by temperature changes. But epoxy resin bond-coats are not a problem because of tbe thinness of tbe coat (a reason for keeping bond-coats thin). For epoxy Dlortar and concrete always use epoxy resins tbat have higb extensibility, thus allowing tbe material to stretcb and relieve differential stress for thick coatings. When fillers are added to epoxy resins and aggregates are used in mortars and concretes, the "dilution" effect produces a lower coefficient of expansion in tbe total mixed material and provides considerable relief. Epoxy resins CUre with almost no shrinkage. Low shrinkage is an advantage in repair work, but can present problems for repairs to newly hardened concrete that has not yet reached a stable condition of volume. Using extensible epoxy resins allows the differential stress to be relieved. Temperature limitations Few epoxy resins cure satisfactorily at temperatures below 50 F and almost none below 40 F. Do not attempt repairs using epoxy resin at temperatures below 50 F until a test application using tbe proposed materials has been made at the expected temperature, and test cubes Or cores indicate that the desired strength is achieved. Avoid high temperatures (generally those above 85 F) hecause increased temperature increases the rate of hardening and makes it difficult to mix, place, and fmish the epoxy resin material before it hardens. At higher temperatures, uSe a material with a longer pot life, mix it in smaller batches, and place it in shallow lifts. Neve, use thinners with epoxy resin. SAFETY DURING EPOXY RESIN REPAIR OPERATIONS • Avoid contact of the epoxy resin or solvents with the skin. • Wear protective clothing, including gloves and goggles. • Apply protective creams to exposed skin areas. • Ventilate indoor repair areas, otherwise fire and explosion are hazards from tbe solvents used in cleanup. • Prevent human contact with solvents. • Immediately remove epoxy from skin by washing with soap and water, but never use solvents. REPAIR WITH EPOXY RESIN MORTAR AND CONCRETE Mortars and concrete are site-mixed with only epoxy resin and aggregates, using fine aggregate for mortar and using both floe and coarse aggregate for concrete. Both mortars and concretes make exceUent repair materials but their cost needs to be justified for repairs other than for small areas and for areas subject to severe service conditions such as cavitation, high abrasion, excessive point loadings, chemical attack, and similar conditions. Normally epoxy mortar is used for repairs less than 1 1/2 or 2 in. deep and epoxy concrete for deeper repairs. 94 MANUAL OF CONCRETE INSPECTION Materials Use epoxy resin binders specially formulated for the intended use, normally the more extensible materials. Use hard, dense, clean aggregates, rounded or cubical in shape, and completely dry. Table 11.1 gives reconmlended aggregate gradations. Coarse aggregates larger than 3/4 in. are seldom used. For epoxy mortars, use the coarser gradations of fme aggregate for deeper repairs and the finer gradations for shaUow repairs but material passing tbe No. 16 sieve is the coarsest that should be used if neat feather-edging of the mortar is required. The epoxy mortar aggregate gradations shown are those found to give optimum results in producing nonsag mortars. Many other gradations are successfully used, some even consisting of uniform sized aggregate. Epoxy resin mortar proportions normally are 1 part epoxy resin to 4 to 7 parts aggregate by weight. Epoxy resin concrete proportions normally are 1 part epoxy resin to 6 to 10 parts aggregate by weight. With the aggregate gradations shown in Table 11.1, equal parts of fine and coarse aggregates are used. Use the maximum quantity of aggregate practicable, but never more than will be completely wetted by the epoxy resin and will have all voids fiUed when the mixture is compacted. Surface preparation In general, prepare the surface by fonowing the requirements previously discussed for other structural repairs. Outline the repair area with a saw cut, 1/2 in. deep for mortars and I to 2 in. deep for concrete. If practicable, make sure the surface of the base concrete is thoroughly dry, otherwise, a moisture tolerant epoxy resin must be used. Compressed air lines used for cleaning and drying the surface must be equipped with adequate water and oil separators. If possible, base concrete and ambient air should be at moderate temperatures (60 to 80 F) both before placing the epoxy resin material and during cure. Preparation, mixing, and handling Condition the oven-dried aggregate and the epoxy resin to a temperature between 70 and 85 F before mixing. Mix the epoxy resin separately before adding aggregate, being sure to add the curing agent component to the epoxy component (following formulator's direction) because using the reverse order may cause difficulties. Mix ttte epoxy resin with mechanical equipment such as a low speed pneumatic or spark. proof electric drill with a paint mixer or propener attachment. Mixing must thoroughly blend aU of each component and must be at a low speed to prevent entrapping air bubbles. Then transfer the mixed epoxy resin to larger mixers or containers, and add the aggregate. The resin-aggregate mixing Dlay be done by hand in a large pan or mortar box or in a mechanical mixer but ·horizontal shaft mortar mixers are preferable. Power mixers using disposable 5 gal. pails mounted at an angle on a power drive base are often used. For epoxy concrete, add aU of the coarse aggregate to the epoxy binder first to get better dispersion. Continue mixing until aU aggregate particles are completely coated, Dump the batch as soon as mixing is complete and spread it On the repair area to Dlinimize temperature buildup. Immediately prior to placing epoxy resin mortar or concrete, coat the surface of the base concrete with a thin layer of epoxy resin binder scrubbed into the surface. Avoid pools of liquid material. Delay placing of the mortar or concrete until the bond-coat becomes slightly tacky, but placing must be completed before the bond-coat hardens, If the bond.coat does harden, lightly abrade the surface, then apply another bond-coat. Place the epoxy resin mortar and concrete in layers 2 in. thick or less, with a slight delay between layers. If excessive beat buildup occurs, lengthen the delay. Consolidate tbe material by band tamping to eliminate aU voids. Screed and finish the same as for other mortar and concretes. Never use a thinner to aid in finishing. Use wood floats and steel trowels for fmishing. If sticking is a problem, wrap steel trowels with polyethylene or use special plastic trowels. Where repairs should blend with the surrounding concrete, the glazed surface can be removed by light sanding or sandblasting of the hardened repair. Some choice of color in epoxy resins is available and a material can be chosen to match the surrounding concrete. Space the injection ports the sanle as the depth of injection required from that surface. There must be sufficient distance between ports to assure that the liquid injected into the port will flow through the full deptb of the crack before flowing out of the next port. InJection After all epoxy resin used to seal tbe crack and to bond tbe entl)' ports has hardened, mix and inject the epoxy resin grout, preferably during a time of day (or night) when the concrete is coolest and tbe cracks are widest. However. do not exceed temperature Iintits of the epoxy resin being used. If the crack is vertical, start injecting into the lowest port until it oozes out of tbe port above. Then seal the lowest port and inject into the next port. Continue in this manner until complete. Witb horizontal cracks, proceed from one end of the crack to the other. Inject horizontal members from the underside, if possible. possible. The crack is considered full if pressure can be maintained. If this is not the case, the grout is still flowing into the unfilled crack or leaking out. Do not apply a pressure so high during injection that the member is damaged by hydraulic action of the fluid grout. After the resin has cured, rentoVe all fittings, and then grind and repair the surface as necessary. Sometimes the gravily met/1(1Ii of epoxy resin grouting may be used on horizontal surfaces, such as floors and decks. An epoxy resin of vel)' low viscosity is allowed to penetrate into the cracks by gravity. If the crack penetrates through tbe entire depth of a slab, seal the underneath side of the crack to prevent tbe grout from running out. Cut the top edges of tbe cracks to a V -shape approxinlately 114 in. deep. blow out and clean it by oil-free compressed air or by vacuuming. The initiation of Phase 3 requires the relocation of the Runway 33 threshold to allow work to proceed within the Runway Safety Area (RSA). There was a great deal ofdiscussion at our last construction progress meeting concerning the process of accomplishing the threshold relocation. A.A.T.I.' s role in that process is key and I regret that A.A.T.I. was not represented at our meeting ofSeptember 17, In light of that fact, please contact Jim Gardner and Alvin DeVane to coordinate the threshold relocation. Our discnssions concluded that the runway must be closed while the markings are being changed and that A. A.T.I. has the responsibility to close the runway and issue the appropriate NOTAMs. r'tk ')..b It is anticipated that H.B. Zachry, Inc .. I observed shrinkage cracks in the portland cement concrete pavement on the west paving lane of Taxiway B at stations 209+75, 210+57,211+56, and 213+00. This condition is unacceptable, therefore I am directing you to remove and replace the defective work. As a minimum, you must remove and replace each cracked one-half panel and saw (and seal) a dummy contraction joint across the adjacent paving lane to prevent reflecti ve cracking. Also, each panel to be replaced shall be doweled into the adjacent pavement. Tbis work must be completed and allowed to cure for 7 days prior to the initiation ofPhase 3. Distribution ofMeeting Minutes from the July 9,1998 and July IS, 1998 Meetings Tex Schmidt distributed the meeting minutes. 2. Comments on Draft Meeting Minutes from the July 30, 1998 Meeting Tex Schmidt stated that copies of the draft meeting minutes had been faxed to Jim Gardner, Jim Pierce, and Zane Ryan prior to the meeting. There were no comments On the meeting minutes. 3. Contractor's Quality Control Program and Testing Plan URSG informed the contractor that, from now on, 4 inch diameter cores will be required for thickness determinations. Julian Wise reported that HBZ is executing their QC program. Tex Schmidt cautioned HBZ to continue to do so, and to display control charts in Julian's trailer. 4. Project Schedule Majed Limam distributed copies ofthe two-week lookabead and explained the the proposed activities. HBZ requested to be allowed to proceed with placement of the base material prior to the 7-day curing period. URSG agreed, provided that the results from all acceptance testing was complete and acceptable. Phase II concrete is projected to begin in approximately 2 weeks. HBZ requested permission to again attempt to place dowel bars into the plastic concrete rather than drilling into the curing concrete, utilizing a new method that will eliminate misalignment and edge slump. URSG agreed ON A TRIAL BASIS ONLY. 5. Construction I Shop Drawing Submittals bv Contractor Tex Schmidt distributed a summary log ofthe Contractor's submittals to date and their review status. No submittals are currently outstanding. 6. Requests for Information (RFI's) Tex Schmidt distributed the updated RFI log. No RFIs are currently outstanding. 7. Coordination with FAA Tower No problems were reported by the FAA, Owner, AATI, Contractor or RPR. Alvin DeVane requested information about modification ofthe control panel in tbe tower. Tex Schmidt responded that he wasn't sure if there would be any modifications required and would check into it. DBE Participation Tex Schmidt asked the Contractor if they were going to meet their DBE goa\. HBZ indicated that trucking subcontractors will help them achieve their goal. 9. Other Items Tex Schmidt gave Jim Pierce approved Change Orders 1&2 and Pay Request No.3. Tex Schmidt provided Jim Pierce with a copy of Chapter 5 ofAC 150/5300-13C and stated that driveway 4 has a 5% grade and it does not meet FAA criteria, and that the proposed project would not change that fact. The entire driveway would have to be rebuilt to improve the grades, and it still would not meet criteria. Jim Pierce decided to allow the driveway to be built as currently planned. 10. This lack of timeliness has been chronic and resulted many problems. Most recently the contractor, H.B. Zachry, was faced with the choice ofeither delaying the placement of base material or proceeding without the knowledge that the lime-treated subgrade was compacted to an acceptable density. Reports from tests performed on August 13 are dated August 20, and were not received in this office until August 28. This lack oftimely performance could potentially result in either delay claims or claims for extra work. It is simply unacceptable. Our client, Mr. Jim Pierce, Jr., P.E., who is the Assistant City Engineer for the Town ofAddison, has also expressed his concerns about Maxim Technologies' lack ofresponsiveness, to the point that he is considering contact with the president ofyour firm. Please contact me with your assurances that we can expect test reports in a more timely manner (Le. within 48 hours), or URS Greiner will have no choice other than to terminate your contract. Comments on Draft Meeting Minutes from the July 30. 1998 Meeting Tex Schmidt stated that copies ofthe draft meeting minutes had been:faxed to Jim Gardner, Jim Pierce, and Zane Ryan prior to the meeting. There were no comments on the meeting minutes. 3. Contractor's Quality Control Program and Testing Plan URSG informed the contractor that, from now on, 4 inch diameter cores will be required for thickness determinations. Julian Wise reported that HBZ is executing their QC program. Tex Schmidt cautioned HBZ to continue to do so, and to display control charts in Julian's trailer. 4. Project Schedule Majed Limam distributed copies ofthe two-week look ahead and explained the proposed activities. HBZ requested to be allowed to proceed with placement ofthe base material prior to the 7-day curing period. URSG agreed, provided that the results from all acceptance testing was complete and acceptable. Phase II concrete is projected to begin in approximately 2 weeks. HBZ requested permission to again attempt to place dowel bars into the plastic concrete rather than -drilling into the curing concrete, utilizing a new method that will eliminate misalignment and edge slump. URSG agreed ON A TRIAL BASIS ONLY. 5. Construction I Shop Drawing Submittals bv Contractor Tcx Schmidt distributed a summary log of the Contractor's submittals to date and their review status. No submittals are currently outstanding. 6. Requests for Infomlation (RFI' s) Tex Schmidt distributed the updated RFI log. No RFIs are currently outstanding. 7. Coordination with FAA Tower No problems were reported by the FAA, Owner, AATI, Contractor or RPR. Alvin DeVane requested information about modification of the control panel in the tower. Tex Schmidt responded that he wasn't sure ifthere would be any modifications required and would check into it. DBE Participation Tex Schmidt asked the Contractor ifthey were going to meet their DBE goal. HBZ indicated that trucking subcontractors will help them achieve their goal. 9. Other Items Tex Schmidt gave Jim Pierce approved Change Orders 1&2 and Pay Request No.3. Tex Schmidt provided Jim Pierce with a copy of Chapter 5 of AC 150/5300-I3C and stated that driveway 4 has a 5% grade and it does not meet FAA criteria, and that the proposed project would not change that fact. The entire driveway would have tc be rebuilt tc improve the grades, and it still would not meet criteria. Jim Pierce decided to allow the driveway tc be built as currentJy planned. 10. Meeting Minutes Leslie Sagar distributed copies ofdraft meeting minutes from the July 9 and July 15 meetings. There were no comments. 2. Contractor's Quality Control Program and Testing Plan Leslie Sagar reminded the Contractor that they are responsible for following their Quality Control Program and Testing Plan. Julian Wise stated that he had not received any of the required charts on the P-501 Concrete from Terra-Mar. Majed Limam stated that he would contact Terra-Mar. 3. Project Schedule Majed Limam stated that Phase 2 is underway. The asphaltic concrete has been milled and the base material has been removed. The 30-inch RCP under Driveway No.9 has been completed. Majed distributed copies of the two-week look ahead and explained that the proposed activities include excavation of the subgrade and lime stabilization. All work will be performed during the day. 4. Construction I Shop Drawing Submittals by Contractor Leslie Sagar distributed a summary log ofthe Contractor's submittals to date and their review status. There were no revisions since the last meeting on July 15. 5. Requests for infonnation (RFl's) Leslie Sagar distributed the updated RFI log. Three RFI's had been submitted since the last meeting on July 15 and are still under review. Leslie Sagar stated that responses would be provided in the next few days. 6. Coordination with FAA Tower Julian Wise and George James both indicated that there had been no coordination problems with the Tower. 7. DBE Participation Leslie Sagar advised Majed Limam that H. B. Zachry had not yet met the DBE goal for the project. Majed indicated that they were still hoping to increase the DBE participation. The PCC concrete ranges in depth from 12 to 18 inches and is approximately 11 ft. wide and 291 ft. in length within Phase·2. George indicated that it may extend even further into Phase 3. Leslie Sagar stated that she would go look at the area immediately following the meeting. b) Jim Pierce stated that the Town ofAddison would not be issuing a permit to H.B. Zachry for a concrete batch plant. 9. Meeting Minutes Leslie Sagar stated that draft minutes from the July 9, 1998 meeting would be taxed to everyone at a later date. 2. Contractor's Quality Control Program and Testing Plan Leslie Sagar reminded the Contractor that they are responsible for following their Quality Control Program and Testing Plan. Jim Gardner submitted a letter to Leslie Sagar regarding Terra Mar and regarding the concrete that had been placed which exceeded 95 degrees Farenheit in temperature. Leslie Sagar stated she would review the letter after the meeting. 3. PrQ;ect Schedule Majed Limam distributed copies of the two-week look ahead and explained the proposed activities. Phase 2 will start on July 21. The "transfer" from Phase I to Phase 2 will occur in the morning. Wayne Crabtree stated that barricades would be set and in place by the evening of July 21. The Contractor will be working during the day for the next 3 to 4 weeks. Tenants at Driveways 9 and to will use Taxiway F, and tenants from Driveways I ana 2 will use TaxiwayC. AATI is responsible for notfying and coordinating with the tenants. The Contractor will notify AA TI when Taxiway F can be used, and AATI will be responsible for notifying the Tower. 4. Operatjng Procedures Leslie Sagar stated that URS Greiner is concerned about the lack ofa clear operating procedure regarding runway closure/opening. Jim Gardner and Wayne Crabtree also expressed concern and asked that the procedure be covered again prior to future night work activities. 7. Coordinatjon with FAA Tower Al Devane reported that he was pleased that the stockpile had been removed and there were no coordination problems during the past week. DBE Participatjon Leslie Sagar asked Jim Gardner about the status ofupdated DBE infonnation. Jim Gardner advised that revised information had been submitted to Tex Schmidt on July 9. 9. Contractor's Pay Request Leslie Sagar stated that she had received the Contractor's pay request. Julian Wise stated that he had reviewed it beforehand. Leslie Sagar stated that she would fax it to Julian for his review/confirmation prior to her signing it. 10. Other Items a) Wayne Crabtree announced that he is leaving H. B. Zachry, and that July 17 will be his last day. Jim Gardner stated that he will be in charge and that they planned to bring George James into the project. Al Devane stated that he would give George James a tour of the Tower and a briefing on proper radio procedures. b) Three inlets have been completed (two are backfilled and one still has forms in place). One manhole remains to be constructed. c) Leslie Sagar advised that Change Orders 1 and 2 are signed and will be forwarded to Bruce Ehly of TxDOT for his signature. d) Jim Gardner advised that they have requested a permit from the Town of Addison for a concrete batch plant on the Land Reclamation site. Ifapproved by the Town, the plant would be dedicated to the West Taxiway project and Jim Gardner will prepare a plan regarding the PSO I mix design for URS Greiner review. e) Jim Gardner indicated that they are seriously considering placing concrete at night due to the high ambient temperatures during the day, and also because they can get the concrete more easily from TX.I at night than during the day. Leslie Sagar advised Jim Pierce that continued night work would have financial implications on the Quality Assurance Testing budget and the RPR Services budget since these budgets had been prepared assuming day work, and night work is more costly. 11. Apparently when it was constructed, Driveway 4 had a grade of approximately 5%, which does not meet FAA deSign criteria for any size or type of aircraft. Our construction plans for Taxiway B require the contractor to remove and replace 15 feet of the driveway, also at 5%. with a grade break at the edge of pavement, which is 17.50 feet from the centerline. Previously, the grade break was approximately 30 feet from the centerline, or 12.50 feet from the new edge of pavement. The plans also call for a reverse crown on the west side of Taxiway B, which will lessen the impact of the grade break. The concem is that, despite the fact that we are not making the problem worse (in fact we're improving the situation). the driveway will still not meet FAA criteria. We could improve upon the design only by reconstructing the entire driveway and adjacent apron. Do you want us to take any further action? We can expect some extra costs associated with the demolition of an old concrete slab. I'll let you know how much additional cost as soon as I get more information on how big it is. Julian said that they expect to lime stabilize the subgrade this weekend. 28 day breaks are coming in now. So far, I've only seen one low break -680 psi, I'll have to do some analysis to see what effect it will have on the pay for P-501. Call me if you need anything.
