Arapaho Rd. realignment has requested some additional speed surveillance through the work zone on Quorum Drive between Edwin Lewis and the RR tracks. There are signs requesting a 20MPH
speed limit, and traffic is continually, significantly, exceeding that. Wrth the workers and construction traffic in the area, this can be very dangerous. Would it be possible to have

responded to locate requests in a timely fashion. One was a SWBell request and one was from Ed Bell Canst. Also, we have a case where TU has a conduit underground that has never been
located (S.E. corner Arapaho & Quorum) for Ed Bell. A subcontractor was trenching in the area and he discovered the conduit before any injury or damage was done. These situations are
not only dangerous, but the are hindering our construction progress. Please advise me as to how we can get more timely and complete locates. Thanks, Jim.

does not have to respond to repair potholes on this project in the future, then the Town is willing to forgo charges to Ed Bell Construction for the ahove responses. Ifthis is not the

temperature extremes. Preparatory Work: For maximum product performance, RICHMOND R-303 should be appiied to a properly prepared concrete surface. Recommendations include; 1.The area
surrounding tile crack should be free of old coatings. oil. grease, dirt and otller contaminants. . 2.Small Clacks should be drilled witll hollow core bit and vacuum chuck. Large cracks
sMuld be slotted. 3.1njection ports should be approved devices spaced at intervals of approximallily 6'. 4.The crack surface and area surrounding the injection ports should be sealed
with Richmond R-306 or Richmond R-616. Methods: RICHMOND R-Ja3 should be applied as follows: 1.RICHMOND R-303 should not be mixed with otller chemicals or altered in any way. Place components

with the exception of using plastic valve box extensions for increased height. Connect irrigation mainline to DCA using Teflon paste and PVC male adapter. 1.11 Female threaded plastic
Spears ball valves with positive T-handle cut off must be installed every 200' of mainline for isolation purposes. House in standard (large) rectangular Ametek plastic valve box and
follow meter box installation instructions for DCA assembly. 􀁾􀁉􀁙an approved size RainMaster Evolution DX-2 stainless steel controller with heavy-duty transient I 􀁾􀁲􀁯􀁴􀁥􀁣􀁴􀁩􀁯􀁮􀀠along
with a stainless steel freestanding pedestal is permitted. Controller must include all necessary hardware to ensure reliable communication and operation with the Town's central control
located at 16801 Westgrove. Installation must include the following RainMaster hardware, purchased only from a RainMaster supplier. DX-03 sensor board, DX-PH phone communication option,
flow meter (size to be approved by owner), and shielded EV-cab-sen flowmeter cable. An approved size Weathermatic 11000 series master valve must also be installed with wiring being of
a solid color distinctively different than that ofthe station wiring. Both flow meter and master valve must be housed in Ametek large rectangular plastic valve boxes. Flow meter able
must be sized appropriately and be of a continuous run; no splices will be allowed except at the point of connection to the flow meter. Connections will be soldered and water proofed
using a 3-M DBY connector. Cab-Sen cable will be installed within continuous W' or larger gray PVC conduit with 6" or larger I-boxes placed every 200' or where 360° of fittings are installed;
only sweeps are permitted. It is the contractor's responsibility to entail the cost of and work in conjunction with Southwestern Bell Telephone to establish a dedicated phone service
and install an interface within the pedestal at each controller location via direct burial cable. Controllers are to be affixed to a Town-approved permanent concrete pad via four (4)
7/16" or larger stainless steel bolts, nuts and washers. All wiring is to enter the pedestal via approved size gray PVC sweep elbows extending at least I" through the pad. Control wiring,
120-volt service, flow meter cable, and phone cables are to be separated with each having its own access elbow. All local and national codes must conform to any and all aspects ofthe
installation. All controllers are to be wired for remote operation with Irritrol RVC units; to achieve this, utilize appropriate size universal pigtail adapters. The entire installation
must conform to RainMaster specifications and be approved by the Town prior to and be inspected during installation. Such specifications will include grounding and pad configurations
and distances of separation from water meter to master valve to flow meter and first fitting. A functional mini-click freeze sensor must be installed at every controller in an approved
location and by an approved method. Consult RainMaster for specifics offreeze sensor installation. 1.13 Use clean and approved topsoil to backfill all pipe to a depth. All heads and
boxes are to be backfilled to grade with clean topsoil. No rocks greater than 1" are allowed. Compact trenches to alleviate settling. Minimal depth ofcoverage is 12". 1.14 All sleeves
will be Schedule 40· PVc. All piping underneath paving. including sidewalks must be sleeved. All sleeves are to be belled PVC pipe. 1.15 Use appropriate and approved PVC solvent. Avoid

cut through the lateral when installing the TU underground, the lateral was repaired with a piece of 15" PVC pipe. We TV'ed the lateral and the PVC pipe has collapsed. Dalton wants to
put a permanent fIX on the lateral and has asked permission to close the road Sunday, for about 5 hours, to do so. He wants to close the whole road, although he could liVe with keeping

of Arapaho Road, a $4 million dollar project, is the highest priority capital project in the Town ofAddison. The reasons for the high priority are the elimination ofa severe bottleneck,
and, more importantly, the new road is needed to service the new DART Transit Center that is currently under construction and ahead of schedule. The Town is committed by written agreement
to provide the completed road for DART when the transit center is completed. About mid Augnst, we discovered a conflict with one ofyour underground duct banks and a storm sewer required
for our project. On August 25, our staff met with your staffin the field and we were told that plans would be prepared and that the relocate would take place in about 2 months. We now
discover that the plans were completed only in late Oetober, and that the SWBell relocate work has not been funded. We find this situation to be unlike the previous cooperation we have
received from SWBeli on this project, and due to time commitments, cannot accept any further delay on the part ofSWBeH. Please take whatever steps are necessary to fund and execute this

all sawed joints that should have been made, shall be made, and to proper depth. In addition, this is to request that you assign a new, competent, General Foreman/Superintendent to the
project, to be present at all times that work is progressing, and with full authority to act for Ed Bell Construction. We also request that a meeting be set up with Steven Priddy, to

on Arspaho Road, 1 made a site visit on Tuesday afternoon November 3, 1998. I walked the entire project and looked at the construction that has been completed to date, The fullowing
list contains my observations and recommendations: • There is a radial crack at each corner of the Existing Arapaho Road I Arapaho Road intersection across the crosswalk. These cracks
appear to be result ofsawed dummy joints not being cut in the pavement Assuming the concrete pavement is the proper strength and thickness, the cracks should be routed and sealed. •
I counted fonr transverse cracks in the right turn lane from paving stations 45+00 to 47+00 on the north side of Arapaho Road, These cracks appear to be result of sawed dummy joints
not being cut within the 5 to 12 honr window as specified by NCTCOG Specifications. Assuming the concrete pavement is the proper strength and thickncss, the cracks should be routed and
sealed, • There is a radial crack at each comer of the driveway locatcd at paving station 55+66.98 to the CarrAm.rica property across the crosswalk. These cracks appear to be result
of sawed dummy joints not being cut in the pavement. Assuming thc concrete pavement is the proper strength and thickness, the cracks should be routed and' sealed, • There is only one
longitudinal sawed dummy joint in the center of the 45-ft. wide northbound Spectrum Drive pavement If cracks develop in this section of pavement they should be routed and sealed. Construction
or longitodinal joints should be located at each lane as detailed on Construction Drawing pol. • There is a section ofpavement in the north lane of eastbound Arapaho Road across from
the driveway at station 63+64.25 where three sawed dummy joints were marked with chalk but were not cut If cracks develop in this section ofpavement they should be routed and sealed.
• The stone riprap that was placed at the outfall of Storrnwater Lines C-2 & C-3 does not meet the NCTCOG Specifications and was not installed as specified on Construction Drawing ST-12.

in the construction area. In order to remedy the situation, Ed Bell Construction agreed to inspect the job each morning and perform the necessary maintenance for the convenience and
safety ofthe travelling public (barricade maintenance should also be included). ' Ifthe Town discovers potholes in need ofrepair, the Town shall notityEd Bell Construction ofthe location
and shall expect repair work to begin within 2 hours ofsuch notice. IfEd Bell Construction cannot, or does not respond within the two-hour period, the Town will dispatch a crew to make
the repair. In this case, the Town will charge Ed Bell Construction for the Town's cost, estimated to be $100 per repair during normal working hours. After hours, labor charges are at

communications, and our meeting in the field on on September 23, 1998, we are concerned about the paving joints made up against the main lanes ofpaving that has been installed to date
between Addison Road and Quorum Drive. We believe these joints do not meet the plans and specifications for the project. At this juncture, we believe there are two options available:
l. Saw cut and remove the suspect joints and repair as recommended by our consulting engmeer. 2. Extend the project warranty to cover the suspect joints for a period offive years.
