reviewed the pay request from Site Concrete and have the following comments: Items 6-10 and 12 should probably be paid, as they are for material, which isn't effected by the time it
took. Item 11 should be rejected beceuse of a poor decision made by Site to order a box that was 7' deep. Items 1-5 are all tied to time. I have documented to the best of my memory the
days they worked and what happened each day. It would be a good idea if the three of us looked at it and established is my memory is accurate. Jam available next Tuesday during lunch

Ebeling Subject: RE: Millionaire Ramp casualty Friday, April 9th: '1NTB staff, after hearing about the ramp failure on the news the night before, visited the airport managers office
around 9:30 a.m. c1NTB staff viewed site with Lisa Pyles and Mark Aceveda. A steel plate was covering the hole. Maintenance crew moved plate away and HNTB staff investigated the extents
of the hole underneath the pavement surrounding the hole. The pavement sounded hollow in an area bigger than just the hole area. Due to a previously scheduled engagement, HNTB staff
left the site and agreed to come back at 2:00 p.m. c1NTB staff viewed the site with Addison Airport staff. After investigating the site, it was agreed the storm sewer in the area had
somehow created a void under the pavement. The plans the airport had showing the location of the storm sewer seemed to ::cnllict with this analysis, but it was agreed that somehow the
problem was related to the storm sewer. The storm sewer was 'ideo taped internally. The tape showed a large failure in the pipe wall that had allowed the surrounding dirt to flow into
the pipe and create the void. <l..fter assessing the situation, HNTB suggested to the airport staff that a contractor needed to be brought in to expose the void imils. We would instruct
the contractor to start at the existing hole, and remove the pavement until we opened it back to "good" Javemenl. HNTB stated that Site Concrete, a local contractor, was already on the
airfield constructing another paving project and t would be a good idea to have them look at the situation. Site Concrete was brought down and it was agreed they would start Jreaking
away pavement Monday morning. Monday, April 12th: HNTB staff met Site Concrete at the apron on Monday afternoon. It was agreed to start digging at the hole and work our way out away
as we found voids under the pavement. After the hole was dug back to "good" pavement, we would remove the damaged 􀁾􀁩􀁰􀁥􀀬􀀠replace with new pipe, backfill with dirt, and a 12" concrete
slab with #5 rebar each way on 12" centers. ::lnce the hole was exposed, it was agreed to break out the pipe to the nearest Joint, put in 45-degree bends, install pipe collars, and fill
in with flowable fill concrete base and then the 12" concrete slab. Tuesday, April 13: Site Concrete called HNTB and said the 45-degree bends would not work and they had talked to Jim
about using a junction box nstead. HNTB commented it would need to be aircraft rated, but a box would be okay to use. Nednesday, April 14th: Jim Pierce sends out an email stating that
Site Concrete will be using a junction box to do the repair, but it may not be here until =riday, because they are having to get an aircraft rated box. Nednesday evening HNTB gets a
call from Site saying they are having trouble with the box. HNTB allows them to adjust the box :0 try and keep progress moving forward. Thursday, April 15th; Site Concrete works with
the junction box fix until 4:00 pm. Dave Foster calls HNTB and asks them to come out and look at the unction box, he doesn't believe it is going to work. HNTB visils the site and after
assessing the situation, directs Site Concrete to 'emove the junction box and use 45-degree bends. HNTB stays on site to make sure pipe bedding is compacted correctly. "riday, April
16th: 􀁾􀁎􀁔􀁂􀀠visits site three times to make sure construction is advancing properly. Paving is completed in the afternoon. It is agreed :hat the barriers will remain in place until
Monday, at which time the apron would be open to traffic. Friday night HNTB sends out 􀁾􀀭􀁭􀁡􀁩􀁬􀀠summarizing the last few days of construction efforts. lAonday, April 19th: 􀁾􀁰􀁲􀁯􀁮􀀠open

will install a Junction Box at the angle point intersection of the 24" pipes. The junction box is precast and comes with cutouts for the pipes to enter the box. The box will be delivered
today. The pipes will be grouted in place good and solid. The junction box will have an invert invert formed on the inside to provide for smooth flow through the box. The box will have
an access manhole on top that will be set at grade. Backfill will be flowable fill. They are having some trouble getting the aircraft rated manhole frame and cover -it may be Friday.
Work can be completed except for that, and thet opening can be covered by a steel plate in the meantime. I am happy with this plan and it will give us a proper fIX. . Jim Pierce, P.E.
